NOTICE REQUIREMENT: For Domestic Services, Sprint will have up to 30 days to complete disconnection. The customer is responsible for all charges 30 days after Sprint receives the disconnect notice. For Customer Provided Access (CPA), 30 days begins once the Local Exchange Carrier's Design Layout Report (DRL) from Customer is received by Sprint. For Non-Domestic Services, Sprint may require a longer period.
FORMS REQUIRED: For written notice of disconnect to be effective, Customer must provide information necessary for Sprint to complete the disconnect, such as a completed Disconnect Firm Order Commitment ("FOC") and Design Layout Report for Customer-provided access . Failure to provide such required information may result in Sprint's revocation of Connecting Facility Assignments ("CFA") from Sprint to the Local Exchange Carrier ("LEC") and Customer will be liable for any resulting charges imposed on Sprint by the LEC.
